Planning Ahead Together
Canvey Conversations: Planning Ahead Together
A welcoming workshop for patients and families following a serious diagnosis
Led by Dr Deepmala Bhagawan
Have you or someone close to you been diagnosed with a serious life-limiting illness?
Canvey Conversations: Planning Ahead Together offers a safe, supportive space to learn more about your condition, talk about what matters most to you, and access support at an early stage.
Whether you're a patient, family member or carer, you're welcome to join us. You will be able to:
- Ask questions in a supportive environment
- Discuss any concerns or worries
- Gain a better understanding of the diagnosis, treatment and care options
- Access support for family members and carers
- Receive signposting to relevant local services
- Arrange follow up appointments where clinically appropriate
This workshop aims to improve understanding, provide reassurance, and help patients and families access the support they need at and early stage.
If you would like more information about the workshops, please email the team at eicb.canveypcn@nhs.net.
