Priorities
- Improving access to primary care services for its local population by implementing an additional virtual GP service called ‘Push Doctor’ to provide a minimum of 65 additional online appointments per week with a virtual GP service between Monday and Friday 6:30pm – 8pm and 9am – 5pm on Saturdays and Sundays.
- Enhancing the care provided to patients with a learning disability to ensure that they have an annual health check and have a personalised care plan developed with a local healthcare professional working in partnership with their GP surgery. These assessments take place in an individual’s home.
- Recruiting and retaining additional highly skilled healthcare professionals to provide an enhanced care to patients closer to home. We will actively promote peer to peer and interprofessional working both inside and outside of the PCN and encourage the staff to undertake a professional analysis of case studies and complex cases to seek to improve future collaborative working to improve the care patients receive.
